Why is this happening!!?

As you can see, her phone is unlocked, but swipe up does not open it. In order to get in, she has to click a notification and when it opens the app, she swipes out to home screen. This keeps happening. 13 Pro Max 15.2

That is not normal - which you obviously know! I have an iPhone 13 Pro and it is on 15.2 with no issues.
Have you tried shutting it complete off (hold Power and a Volume button at the same time then slide the slider to off). and restarting it (Hold power button until Apple appears)?

I have had that happen a few times. Rebooting the phone fixed the problem.

I’ve rebooted her phone a few times and it still randomly does it.

Sadly it looks like it will require an update. I would contact Apple and even if they can't help they will know it is an issue.
