Super quick as to not waste your time: I have an Asus RT-N66R router, which covers my whole home (in 802.11n), and all of my other devices have no problem staying connected. I have a lot of devices that have absolutely no issue. This stupid phone lasts about 5 mins of constant usage before the Wifi performance goes to ZERO despite full bars, making me go to Control Center and toggle wifi on and off. It's driving me crazy. Any advice? I'm on the verge of going and getting a new router.
 
Have you tried the phone on a different wi fi source?
 
I recommend the same app @now i see it recommends.

You'll be able to see if you have data throughput to the router. With this information we can narrow down the issue to the router or iPhone. Yes I know you said your other devices are fine but that doesn't mean the router is treating the iPhone the same.

How are you measuring "performance goes to ZERO"?

What happens if you test right next to your router?

Another tool that you can use is Network Speed Tester. You install the server side on a Mac, and the client on the phone. This way you measure the speeds that your network supports. Tools such as Speedtest measure internet speeds, which is a different beast. This eliminates the internet part of the equation.
 
Recommended settings on your router. N mode is entirely not stable and may be broken.

2.4Ghz
Wireless Mode - Auto (Do not use N)
Channel bandwidth - 20/40Mhz or 40Mhz (only if it is not crowded)
Control Channel - 1, 6, 11
Extension Channel - Auto
WPA2-personal

5Ghz
Wireless Mode - Auto (Do not use N)
Channel bandwidth - 40 Mhz
Control Channel - 34, 38,134. 159
Extension Channel - Auto
WPA2-personal

You may need an app to scan to see how many wireless in your area and pick a channel which does not interfere with yours.
 
Update for everyone, after careful diagnostics, I determined the router to be the source of the issues. After 5 hours of fighting with a new modem and router, I am up and running and my phone reports 6x speeds across the whole house AND no dropped connections! Google Wifi ftw
