Wrong Album Artwork When Copying Songs to iOS

I noticed a lot of people have this problem but I have yet to find a solution.

Problem: Every time I copy songs from my iTunes library to my iPhone 6s, the album covers change to a random album cover when I access the song in the "Music" app on the iPhone.

The covers are correct in iTunes on my MacBook but are incorrect every time I copy them to my iPhone.

FYI, I don't like syncing and have iTunes set to "manually manage my music and videos". This issue started about a year ago.

iTunes: 12.8.0.150
MacBook Pro: 13-inch, 2017, 4 TBT3
iPhone 6s: iOS 11.4.1

Does this also happen when you embed the artwork in each media file before copying? I like to do that for various reasons, and use Doug Adams' iTunes scripts for that.
