Since I have updated to 3.0, my phone is displaying wrong artwork with songs; for example, a KISS album cover appeared on a Sheryl Crow song. I Googled this and tried the suggestion to delete the playlist and then resync the phone with the playlist again, but this problem persists.

Now a new problem with 3.1 -- my Recently Added playlist,, which I created myself as a Smart Playlist to play back songs I just added to the phone, lists totally the wrong songs! It seems to have randomly pulled 125 songs from the front part of the alphabet and put them on the phone instead of the songs in iTunes. I deleted the playlist from the phone and iTunes, recreated the playlist to my satisfaction, and then resynced. Still the wrong songs!

I checked the playlist in iTunes, and it has the right songs. And in regard to the wrong artwork on the phone, it is all correct in iTunes! Anyone else had this nonsense happen?
 
My Recently Played & Top 25 playlist on my iPhone have completely different songs than in iTunes. AND, the artwork on the iPhone is missing.
 
I have exactly the same problem. On iTunes the "recently added"-playlist displays the correct songs, but when I open the playlist on the iPhone it contains completely wrong songs.
Yesterday I had an appointment at the apple store in 5th avenue New York City to get the problem solved. And the guy at the "Genius"-Bar really told me that this was normal and that I just have to live with it..... Genius?

WTF apple? Get it fixed!!!
 
Hate to say it but you are gonna have to restore it. You have a corrupted itunes database on your phone. This has happened to me before and the only way to rectify it it to restore your phone and add music all over again.

This happens when you interrupt a sync--by either stopping a sync in the middle by swiping, or by removing the sync cable during a sync.
