I am making a custom youtube background for someone, and was wondering how people make their ultra custom youtube backgrounds...

The one I was given as an example was

Now I am aware of how to put in the basic background, but I am wondering how one would place links in their custom background?

Help is appreciated, I want to give them something spectacular!

You can't put links onto a "body" background. The only way to put links onto a background is by using div's with CSS. That person is using a "header" image with an "image map".

An image map is a series of coordinates that place hyperlinks over an image without having to split it into multiple images.

You can easily add them using a program like Dreamweaver.
 
i'm not too familiar how youtube works, but i think in order to get a banner like the one in the link you provided, you have to have a certain amount of subscribers/views for youtube to grant you the ability for more customizations. note that that user has close to 80,000 subscribers, over a million channel views and over 10 million video views.
