I have noticed "Siri Suggestions" must be on for this to work.

Doesn't help. If you toggle it off and on again then it works for some time and then stops again.
I filed a bug report, Apple asked for sysdiagnose, I provided that. We'll see.

Thanks for working with apple on this. Here's hoping the next update addresses it.

Response from Apple:

You have Magical Moments disabled by one of your settings on your device. You have predictions that could be surfaced but nothing is being surfaced for one or more of these reasons:

1) You uninstalled the Siri App Suggestions widget from left-of-home.
2) CoreRoutine or “Frequent Locations” is turned off in Privacy > Location Services > System Services > Frequent Locations.
3) Siri Suggestions in General > Spotlight Search.

Seems that missing the widget is the cause of app suggestions not working but I have to test some more.

So if I understand corrently then they've removed the simple "Suggested apps" toggle and are forcing to use the widget instead?

Adding the widget to the Today panel fixed it for me!! I added it then immediately removed the widget and it still works. Plugging in headphones now shows the music app on the lock screen.

if it stops working again then re-add the widget. You might have to have it enabled in widgets for the suggestions to keep on working.

I'm turning the widget back on. This is the only thing I had off. Let's see if it works.

The Siri Suggestions widget is on. I now get suggested apps on the lock screen bottom left when turning on my Bluetooth headphones, and if even wakes the screen like before!

I will report back regarding location based suggestions (Starbucks, Costco, 7-11, etc.)

Edit: Last night I got Apple TV app suggestion in the bottom left -> spot on! works better than ever! It looks like the Siri Suggested apps widget was the key.

I still haven't had a chance to try location based notifications (Costco, etc.) but I may be able to check that today. I will report back.
