I use 2 "standards" and we use a mix some things at work.
When specifying dimensions with a particular orientation then I use W x H.
When specifying dimensions with no particular orientation (e.g. size of a sheet of paper) I use Smaller x Larger (e.g. A3+ paper is always 13x19 regardless of the orientation the image printed).
At work (a photographer's art gallery), we use W x H when spec'ing the size of piece, but my print files always list the size as part of their filenames using Smaller x Larger. This makes finding the proper files in a large folder containing many files for varying print sizes much much easier.
