I'm the type who does a dfu restore (restore with new ipsw file in iTunes), and fresh installation when major updates come. 14.1, 14.2 etc. Know well it is not necessary but thought it works well. I never restore from a backup.

I was considering getting an eSIM for my iPhone 12. I can see my telecom provider charging $ 15 for an eSIM, but I'm a little in doubt if I should pay for a new eSIM when I restore. Is actually in doubt about how it goes. However, I will not pay $ 15 every three months.

Does anyone know how it works?

I have a T Mobile prepaid eSIM and they don’t charge for re-issuance. It’s $15 a month for unlimited talk/text and 2gb data but it’s a biz line so I don’t need data.

I repeat: doing a DFU restore doesn’t erase the eSIM.
The eSIM can only be erased in the settings on the phone.

You should probably still be able to scan the QR code in?

some carriers’ QR codes expire after a while

It's absolutely perfect. Then I will get an eSIM for my new iPhone. Many thanks for the answers, and thanks to you others as well.
