So, quick update, I upgraded to watchOS 6 beta 5 and now the battery usage is back to normal, the watch actually had some real standby time now.

The watch went down from 100% to 89% in 6 hours, which is very good I think. Previously, on watchOS 5.3 it was using 4-5% per hour under iOS 13.

If there was a non having to get Apple involved way to downgrade Watch OS, this would be a great fix, problem is, the next Watch OS beta could break something, at which point you'd have to give up your watch for a few days to get Apple to restore it. I tried WatchOS Beta 1 and had extreme problems, and had to send mine back to get downgraded (was gone less than 48 hours, so pretty fast), but am a bit weary now when it comes to Watch OS betas. Glad to hear it fixed the issue however.

Definitely something keeping the Watch active in the background.

I was a little sceptical, I confess, until I saw this statistic.

Very wary of a move to WatchOS Beta though as my Watch is not a test device.

Series 4 Watch running WatchOS 5.3 and iPhone X with iOS 13 PB4 - terrible battery drain - 10% gone since I took it off the charger this morning (about 70 mins ago).
the Watch app is stating since Last Full Charge:
Usage: 2 hours, 0 minutes
Standby: 2 hours, 21 minutes

I kind of surprised those figures don't match the time it's been off the charger!

Went to bed last night at 11:30 at 98% in movie theater mode and on DND, woke up at 6:30 at 66%.

Yeah watch drain was brutal. It actually compelled me to remove the beta (that and a couple of other issues). I think I’m done with the betas at this point. Watch was dead at 5pm today and that's just not going to work for me.

Guess watchOS 6 beta is the way to go. Here is my data. Apple Watch series 4.

I read in Reddit a possible fix: disabling location services for the Weather app on the iPhone.

Settings (on the iPhone) -> Privacy Services -> Set Weather to Never.

After restarting the AW, it’s working for me.

Settings (on the iPhone) -> Privacy Services -> Set Weather to Never.

After restarting the AW, it’s working for me.

Hmmm... I never use Weather app so my location was already set to Never for Weather on the phone... and yet I still get faster than normal battery drain on my watch (Series 4). I used to be able to end the day with about 45% of battery. Now, around 10PM, I get a 10% warning.

Crazy drain here as well.. Never had to charge my AW4 twice in a day!

Have turned off a bunch of notifications (weather app and raise to wake still on ), and background refreshes, and fully charged it again.

Lets see how this fares now!
 
So, it's been an hour and the watch is already at 96%, and with no activity from my side.
