I get the point you made, but would like to point out that my 2018 MBP is one of very few USB-C laptops that will happily charge off a generic 5W brick, slow as that may be, instead of just spitting out a notification about not enough power and refuse to start charging.What's next, Macbooks not coming with chargers? "Good thing I have this 5W brick!" *facepalms*

I don’t understand. It seems pretty simple to me. Put everything that the item needs to function out of the box in the box. If they need replacements, then they must go and pay for them. Apple will make money on the replacements and customers will have what they need to make the item work when they first buy it. Win win. Yes, that should be the way it is when you buy your device at the store. And if you order one, you have to check what color, what storage capacity etc. you want or if you want AppleCare. There could easily be a place to check if you need a cable. This is something that does not have to be difficult.We should be able to get a free charger with purchase. It should be an option.

When I traveled to Europe (London, Paris) over the summer, I found that USB-C outlets had become practically ubiquitous everywhere I went. Every hotel had multiple USB-C outlets in the room. Several cafe's had them. I didn't need to use a power adapter once.
Given this, it makes sense to me why the chargers are not (or less) needed in the EU now compared to the US.

Apple is allowed to provide a voucher or coupon. The fact that Apple charges for it, but doesn't include it in any form, tells you about their priorities.

The logistics of packaging them, shipping them separately to each country, keeping track of them, having inventory available and having staff at stores or to set up shipping of the power brick more than likely ends up costing Apple more than just throwing it in the box as the iPads are packaged for shipping them directly from China.
The removal of the USB chargers is a result of the EU's mandates on waste and recycling, Apple wouldn't have fought them if it was cheaper to not include them as part of the package.

There’s no additional logistics. Apple Stores already sell them for $19 each. Vouchers can be provided for redemption in-store only.
If Apple wants to think differently, they can even make the vouchers valid at any Apple Authorized Reseller.
