After much tapping, I finally got it to work on news.yahoo.com. Haven't had any luck on other sites. My finger is now numb from repetitive tap syndrome.

RTS.. very sad desease. There are some excellent therapies out there now for you!
 
It happens quite a bit with me. It's just holding the screen down, usually on a button (play the log stacking game for a lot of ACTION popup).

Why is it a big deal again?
 
I think this is a representation of how pathetic some iPhone news is. We want to talk about it so much, but we run out of things to say because Apple hasn't given us very much.

Agreed. This is old, I've seen it when I tried to change the volume on an iTunes remote control web app I used even before the 1st iPhone bug fix. Held my finger above the volume bar for a few seconds and there it was.
 
The answer is given in WWDC's keynote on iPhone web dev.

At 33 min and 06 seconds in the WWDC 2007 "Designing Web Content for iPhone" (this is available for free for anyone who has an Apple Dev Connection account).

Action means that the region is either :
- an Image map area (remember those old school hot zones in a big image...)
- an elements that will trigger "mousemove, mousedown, mouseup or click" javascript events

There you go ! That explains why you don't see it everywhere.
