I have the Pro, latest one. I noticed sounds still coming through, soon as I went from one that had the mute physical button on the side.

same with Xs. maybe an iOS thing.

Just FYI
I have iPad Air 2 (no physical mute) and haven’t experienced alerts coming through when I have the “Ringer and Alerts” slider all the way down in settings.

Also not experiencing this on iPhone X.

iOS 11 or 12.
 
I just tried reproducing this on an XS Max playing a YouTube video in Safari. There was no noise when the volume was fully reduced.
It happens in the Tweetbot app and ******* on Safari & other websites/apps that don’t use the default video player.

I have experimented and set both to 0 and off. and still receive media notifications with sounds. In meetings I don't bring my iPad in anymore. Unless i'm an idiot or doing something wrong. Spoke to a tech as well, they have no comment - submit a bug report.
Ultimately the silent/vibrate and/or DND is more for that kind of thing.

Have you tried the mute button in control center? Sorry, my iPad isn’t around at the mo and I’m not too familiar with it. Probably stating the obvious but just wanted to try and help!

From memory you can’t actually turn ringer (notification) volume down to zero, the minimum is one bar, and then you have to use the mute switch
 
My X is completely silent with the volume turned down... and as always been.
Tested on recorded videos I have made, tested on YouTube videos, tested on Facebook videos, tested on Spotify (not video) and Apple Music (not video) and again when the volume is turned right down, you get zero audio through the speakers.

What phones have you tried this on to give you the title; "iPhone X/XS/XSM: no mute?" because I'm almost certain you've not tested this on all those devices.

It may be a glitch with the phone your using, which model that is you don't specify, but if there's a problem try a software update/re-install or take it into an Apple store. If it's a new phone, take it into Apple and they will sort this issue out as it's not widespread at all, and we aren't all fanboy's on here for what it's worth.
I most certainly tested it on all of those devices as well as on the iPad Pro 10 inch, iPad Mini 4, and 7+. It’s a bug.

Sounds like it probably mostly comes down to what was mentioned in an post earlier:
This happens on the 7/7+, 8/8+, X and XS it seems. I already had this happening it's very rare you turn the volume completely down but you still hear sound coming from the speaker if you hold it close to your ear, it's like iOS didn't completely turn down the volume. It's probably a Software bug so send Apple feedback in hope they can fix it.

I couldn't hear anything on my X on iOS 12. Tried music, youtube and a video from the photos app.

I do however notice that if I unplug headphones even when nothing is playing I get a "pop" sound. I think the headphones are on even when nothing is playing.
 
But the rest of us have devices that mute.
Doesn't quite seem the rest given that there are some others that have mentioned coming across some similar type of things. And even if it's just the OP, that still means that something isn't right in OP's case.

Have you figured out how to mute your device, or do you need help?
 
Could have answered the question at the start by saying you don’t need help instead of continue this thread of answering questions with questions.
Seems like I did that with my question of why I would need help, (along with the thread itself where I didn't ask for help with this, where the poster that asked me about it has participated and in earlier as well). But with someone thinking that I did, I was also curious to find out what made them think that.
