Has anyone been able to fix this? It still happens for me on iOS 17.3 and watchOS 10.3. Medication notifications always go to my iPhone first, and then watch 15 seconds later. Before iOS 17, medication notifications behaved like every other notification: if I wasn't using any devices and wearing my watch, my watch would vibrate. If I was actively using my iPhone, the notification went to my phone instead.

Still suffering this issue. The only things I can conclude are:

1: Its not a bug, and that its the new way of notifying from the Medications app (why they would change it to this is beyond me).

2: It is a bug and it's going to be one of those bugs that takes a long time to get addressed.

If it is a bug, then the only thing I can suggest is to submit feedback to Apple (hopefully the more people that do this the sooner it will be fixed).
 
I've just updated to 17.4 and this bug appears to be fixed. I've just got a medications alert, and it sounded on my Apple Watch only, rather than on my iPhone first, then followed around 15 seconds later on my Apple Watch.
