It's a built-in calendar widget, part of HTC Sense; I'm not sure that it's available on the Android Market.

My setup on my Galaxy S3. Currently running Task's AOKP 4.2.2 ROM.

Lockscreen using WidgetLocker. And my homescreen. I changed the icons using Icon Changer free and the icon pack from MetroStation.
