Ha, ha...sorry man! It was breakfast time...lol.
You can find the theme over at ModMyi (AWSUM-1 posted the link a page or two back) and you should just be able to go into the theme folder and pull out the files you'll need. I've actually never pulled just the dialer from a theme, so I don't think I'll be much help on which files you'll need, but I'm sure others here can help with that.
Taking a break
Go into theme/Bundles/ and pull the com.apple.mobilephone folder.
Could also be a totally separate folder in the themes d/l. Some set the phone, SMS, etc separate from the themes icons.

Not a fan of the silver ring around app store icons. IMO it takes away from the true beauty of the Upojenie icons.

Here is my home screen that is a random hodgepodge of themes but I love it! Still working out some kinks, but for now,

Yeah, I found that mask was a distraction from the rest of the set (plus the issue of it theming already themed app store icons), so I deleted it for my own setup. If I can't find or make a proper themed icon, the app goes into a folder so I don't have to look at it.
If you're heart's set on using the mask, try taking the icon bundles for apps you don't want masked and put them in a theme of their own. Keep it activated ABOVE Upojenie in WB--I think that will work, though I'm not positive.
As for the rest, I like what you did with the wallpaper, and yes, I think
a different dock is in order. Darker/more reddish wood perhaps--that tends to set off both themes you're using well, especially Upojenie.

I'm on 4.2.6 now and it's a mess coming from 4.1. Does anyone know if there's a list/thread anywhere that tells the changes that 4.2.6 had? Here's a few I've noticed right away.
Camera, photos and maps icons won't change.
There is a funny shadow under the icons.
My folder outlines aren't working.
The switcher/folder backgrounds aren't working. On 4.1 I just had to deletethe switcherlinen thing, but on this one here are like 4 switcherlinen files. Do I delete them all?
Any help is much appreciated!!!
Other than the mentioned problems, I absolute love being on verizon now. I actually get full 3G where I live AND work =)

I see what you're saying fantasticgraphic but that icon mask does come with the theme. It is optional and I suppose I could try without but I hate that the app store icons don't have that 3-dimentional look like the rest of the upojenie icons do.

For the camera make a folder and name it com.apple.camera and put the camera icon and name it icon.png/icon@2x.png
Maps is Icon.png/@2x.png. (if that still doesn't work try adding the ~iphone before .png. Maps is weird on the att so try diff things)
Respring thru winterboard (uncheck then check your theme; hit respring)
Thanks on the wallpaper. I forgot I had this dock stashed away so I think I'll go with it now;
As for the icon mask, as I said to fantasticgraphic, I hate not having themed icons for app store icons and although your suggestion to throw none-themed/masked icons in folders is appreciated, a lot of them I use regularly and want them on the springboard instead of buried in a folder. I really liked the mask that the Epic HD theme applied but I much prefer the custom icons of Upojenie. Such a dilemma!
SoundForge or whoever should've made the appiconoverlay/mask clear (not transparent) instead of the silver

Is that your opinion or you saying that one exists? Cause I would be down for that for sure!
NO NO just my opinion. He did release a PSD but you have to have a degree in Rocket Science to figure it out!
