Mine is the stock dialer that comes with the theme, "Laguna HD iPhone 5".

It is simply provided with the full theme.

Oh, same with the cell/wifi bars.

what Cydia tweak is it that removes the icon labels like this? I have the "No Undocked Icon Labels" and "No Docked Icon Labels" listed in Winterboard but they do not work for some reason

I submitted it to the modmyi repo yesterday afternoon. I'll update this post when it's available in cydia.

Winterboard or springtomize. Make sure the no docked labels and undocked are at the top in winterboard.

Iconoclasm is probably te best way. But there are other ways like customgrid.

No problem. I haven't tried CustomGrid myself because I'm just used to Iconoclasm. But Iconoclasm on the iPhone 5 acts weird when trying to create a custom layout, for me it does anyway. Let me know how you like CustomGrid.

I use CustomGrid 2 and it has worked flawlessly for me (having been specifically designed for 6.0).

You could do with installing Boover to realign your icons notification badges, they look off to me.
