What's the name of the widget you are using here? I like it!

Its the the old revi-Dx theme I'm pretty sure. I had the file left over from my old 3GS and I tried to see if it worked on my iPhone 5, and it did, so I ended up keeping it. The only problem is that the wallpaper gets cut off at the bottom, and I can't figure out how to fix that.
I can send you the theme file if you want, just let me know.
