Here you go.
I kept the LS and Status separate, they're both in the zip. The Ai and Ps files are in there too in case you want to move things around.
The battery isn't perfect but I haven't gotten around to fixing it yet (if you do, let me know).
Look for "Slider Remover" in Cydia it works thru Winterboard to remove the "slide to unlock" text, the slider background or the slider itself.
Also make sure that the image is made for iP5 vs iP4... An iP5 image will be taller/ longer to fit the bigger screen.

I don't have a 4S version but code-wise, there shouldn't be much of an issue other than changing the Layer section of the html from:
#Layer {
width: 320px;
height: 568px;
position: absolute;
top: 0px;
right: 0px;
down: 0px;
left: 0px;
}
to:
#Layer {
width: 320px;
height: 480px;
position: absolute;
top: 0px;
right: 0px;
down: 0px;
left: 0px;
}
The original Illustrator and Photoshop files are included if you want to mess around with it too. Sorry, I don't have a 4S to test it on.
Please note: I barely know what I'm talking about. I could be wrong on that code stuff above.
Yeah, I'm not a fan of it but it's the only one I found on a quick search that left a file up for more than 7 days. I used to use MediaFire but now they only keep links for 15 days. I like WeTransfer but they don't say how long they keep the file up.

I don't see that "SB D0ink" widget in Cydia. Can you please point me to it, and if it's not a Cydia install, could you please explain how to do install it manually? Thanks!
I really like your lock screen, I saw someone post it on an iFans forum and they only provided a DL link to somewhere else. Been trying to find the creator ever since, great lock screen!
I know the creator of the original (discordante) is very adamant only allowing people to download his lockscreen via dbar.
