Here you go. It's the d0ink widget. The worded clock is part of it too. Someone more knowledgeable than me can probably tell you how to have just the bottom weather part if that's all you want.

Lock screen uses Rising Bars from the patrickmuff.ch/repo repository, and Unfold.
Homescreen has the theme 'iShady HD' (I think it's about $2), it has a really good automatic icon creator that works well on any app.
I've also got FolderEnhancer installed so there's multiple pages in the folders at the bottom.
The wallpapers cycle through this album which has 300+ really nice minimal wallpapers, all iPhone 5 sized.

It was an iPhone 4 LS, Can't remember the name of it but I updated it to work on the iPhone 5 and changed the clock for an almost identical Swiss Rail clock plus the unlock text.
