Where did you get those fonts from? I checked cydia, google, and iphoneruler.net with no success.
EDIT: NM. I found them in the Elite 6 theme.

The wallpaper (Bubbles Pro) is from the LivePapers app in Cydia. The app that lets you customize the colors, bubble size, bubble count, etc...
The status bar theme is a mix between the iOS 7 theme (specifically the signal dots) and Motif.
Hopefully this helps some others... had some fun tracking some of these UIImages down!

Will soon update the () lock screen to include the old big style too
After playing with the UniAwLS, modifying one of Mone's overlays from Dbar, swiping a glass mini player (also Dbar), making a glass Atom handler, and combining that with Sublimity icons, Motif page indicators and Boover Badge, glass dock from Sarif, and Schnedi's Degraded statusbar elements...this is my setup.
