The homescreen widget is called, for some reason best known to itself: iW RM2 Sch, and you can get it here, http://www.mediafire.com/download/5kp9e8tw4pueod2/iW+RM2+Sch.zip
You'll have to use iFile or similar to install it in the Widget library, unfortunately, as it's not on Cydia (as far as I know).
I used Iconoclasm & Bigify to re-arrange & resize the icons
The lockscreen is Convergence, running Another Theme LS in parallel. I also really like the Patrick Muff clocks for Cydget, attached is the lockscreen running ColorFlow, which is another really nice minimalist LS clock.
