No not stock, it's an SBSettings theme called iRetina

iRetina has square toggles and yours has the IOS 5 rounded toggles. How did you get the rounded toggles?

It's just Springtomize 2, under the Status Bar section! Custom carrier name. Star is everywhere on google or I used the Japanese keyboard for it!
