Mine didn't go off this morning either!!!!!!!

I've had an iPhone for 3 years now, ever since the 3G came out. I've never had a problem with my iPhone 4 (had it since launch day), and I finally upgraded to 4.2 (or whatever the new software is) and it was set properly, the clock was in the window and it just didn't go off today. Very strange.

I'm a pilot, and I rely on it, so I hope it's just not some new bug. I'd be really mad!
 
I had a simular issue yesterday. Luck was on my side as I awoke before the alarm time. I went to check the time and turn off the alarm.

Much to my suprise the iPhone 4 did not respond. I put it to sleep right after setting the alarm and the phone was at 100% charge. It appeared to be dead. So I plugged it in to change and nothing happend, it did not power on and show a low power state or respond at all!

I finally ended up doing a reboot: holding down the Sleep button and the Home button until the boot cycle started.

First time I have had an issue of any type since delivery on 6/23.

This could definitely be a real issue if it's widespread. Multiple alarms might be your best bet for now. Hopefully it's not a bug in the new update. My iPhone 4 is still on 4.0.1 since it's jailbroken, and I haven't had a problem. I've had every iPhone to date, and my alarm has always worked perfectly. I'm really curious if others are having this problem as well. Might not pay to upgrade even when the jailbreak is available. Some other users have posted about issues with memory leaks now with the camera running in the background. Sounds like this is a beta test for 4.2 in November.
 
There's nothing wrong with the phone you most likely didn't turn the Alarm on after you set it (probably assumed since you just got it, it would automatically set). Dial your VM and make sure both volumes are 80% or better. You won't have any issues. Ensure there is an icon in the top taskbar before bed.
 
I had a blackberry before I got my first iPhone, and one thing I noticed about the iPhone alarm is that (if you are new to it) that you have to set the alarm for "everyday". It asks you when you want it to repeat and heres what happened to me first time.

It was Saturday night, I had the alarm set (I assumed for Sunday since with the Blackberry when you set the alarm it will just pick every day or weekends or weekdays) and it asked when do you want this alarm to repeat, so I selected Monday. I naturally assumed that since I had the alarm set in the first place it would work and just repeat on monday, it turns out that it ONLY rang on Monday because I didn't have Sunday selected as well.

Now I have several different alarms and I set them all for every day and just select the time for the days I want.
 
just a tip for the ones who might even get annoyed by a silenced vibrating phone during night times. i just set mine to airplane mode, which gets rid off all types of notifications.
 
This has happened to me several times; on the 3G, 3GS, and 4. I've noticed that during the night, if I receive a text or a score update from the ESPN app, it's almost like it overrides the alarm, and the alarm will start to go off only after I slide the switch and enter my passcode.
 
Mine didn't go off last Monday. The popup was there but the alarm never sounded. First time ever for me
 
I had one time about 2 years ago on my 3G when the alarm didn't go off. I BELIEVE i might have turned the alarm on, done a reboot (possibly after midnight), and even though it showed a clock icon, maybe it didn't "pick up" and use the alarm. YMMV...
 
Might seem obvious, but I've done it before: did you have the alarm sound set to "none"?

Just for an update the alarm has been working correctly since the first morning when it failed to go off.

This was my guess. I didn't see a reply to you from the OP yet. Curious to know if this was it or not.

OP, did you look at the days? Do they all have check-marks on them?

The repeat was set to "Never", but it's not like it went off the first morning and then didn't the next morning. This was for a one-time alarm.

Did you check the "Repeat" setting? It once happened to me but i had the wrong days scheduled and I forgot to change that...
