OK, to clarify: reboot in Safe Boot mode? Then I should turn on startup programs one at a time within Safe Boot? And then reboot every 48 hours or so and add a startup program?

No. Rebooting in safe mode defeats the purpose. None of the startup items will load if you start in safe mode so you won't know if they make a difference.

In the first place, did you actually delete the items in the startup items window by highlighting them and clicking the minus sign so they disappeared entirely, or did you just uncheck the check box beside each item?

Just unchecking the checkbox doesn't help. The unchecked items will still load, just in the background. So you have to highlight each item and hit the minus button to remove them altogether.

Once they are all gone, then boot normally and see if you can run normally.

Then re-install the startup items one at a time, rebooting normally (not safe mode) between each one.

The idea of safe mode is just as a test to prove if launch/startup items are the issue. If it works fine in safe mode, but not otherwise, then you know that is the problem.

So what you would do is start removing those items (I like to print out the Etrecheck report and use it as a worksheet to mark up) half at a time then restart to normal mode and see if things are better. It is just trial and error to find the offender.

Just remove the items to a folder on the desktop and restart to you easily put them back if that item is not the issue.

I am using Alfred on El Capitan with no issues, so I think you are safe to but that back. I think someone earlier mentioned disabling Spotlight indexing. IMO that is not the issue here, and disabling Spotlight will break Alfred since it uses the Spotlight index.

OK. Deleted them all but Alfred from Startup items. Rebooting into "normal" now.
[doublepost=1456199575][/doublepost]OK, just deleted all startup items from preferences except Alfred and restarted in "normal" mode.

Here comes the "at wit's end" part of this whole thing again:

Ran in Safe Mode for 72 hours successfully. Halfway through manually started up three startup items: Flux, Middleclick, and Alfred. All good for the second 36 hours.

Deleted all startup items from System Preferences except Alfred, booted normally. Beachballed in five minutes.

Deleted Alfred from startup items, rebooted normally, and with no startup items running, beachballed after about an hour.

Next steps?
[doublepost=1456209369][/doublepost]I would reiterate two and add a few more observations on Safe Mode: there were rendering issues when changing apps and whenever I used Chrome at all. OS X did not recognize my USB amplifier. Alfred's indexing commands "," to find files, and "in," to find text in files, did not work, and would eventually turn into default searches (of Google). SO there is more difference between Safe Mode and booting normally than just the startup items -- at least on this installation of this machine.

Have you got any old printer or scanner drivers hanging around on your system? That could be the issue I have seen similar before and it's fine when they are deleted.

Safe mode disables graphics acceleration, so you will notice anything graphic related running a little slower. It also disables any third party drivers like those perhaps used by the USB amp. So this all sounds normal.

You are misunderstanding one issue though. There are other places besides that login items section in Users & Groups for launch and startup items. They can be in any of the following folders also. If you look at the Etrecheck report you will see in which area the item resides.

~/Library/LaunchAgents (that ~ is your personal users folder)
/Library/LaunchAgents
/Library/LaunchDaemons
/Library/StartupItems

The fact it works without issue in safe mode for so long really does drive home the point it is one of those other launch items to me.

Excellent, thanks for the correction of my misunderstanding. I will spend a day or two in Safe Boot with my Users and Groups startup items running (manually initiating them) to rule them out as problems. Then, Thursday or Friday, taking on the remainders. Does that make sense as a strategy?

Additionally, Samuelsan, I have had only the same printer for the life of this iMac. How would I check for extra drivers? In "Printers and Scanners" it is the only printer listed.

Yep... that totally makes sense. There is just no easy way to do this other than removing those items and rebooting to normal mode to figure out which one it is.

OK, will do. Right now running in Safe Mode for 24 hours with all System Preferences startup items manually activated. We'll see how that goes for awhile.

What a pain!

Not sure if this info will help, but ...

My daughter dropped her 2012 Macbook Pro. Seemed like a typical case of busted hard drive. Sometimes it would sometimes it would not. Seemed to be a temperature correlation to the crashing or not booting. I still thought it was a bad hard drive. So I bought her a SSD and swapped out the rotating internal. Put the old rotating into a USB hard drive enclosure. Always boots to external drive. So hard drive was bot bad. Works fine except for being slow because running over USB. Will generally boot from internal SSD. But not always. Sometimes can't event "see" the internal ssd.
I took it in to Apple. Genius couldn't find anything. They said they could do more extensive testing over a few days. I sid go for it. Still couldn't find anything. They even redid the CPU thermal paste.
I think its still temperature related with the hard drive cable or subsystem on logic board. As long as the computer doesn't get loaded down, it general works. I "load" tested it. I ran Prime 95 or something like that, that ran the CPU at 100%.
If run from internal hard drive or ssd , the system will crash. If running from external drive (rotating or ssd) & fully loaded , works no problem. Just slow due to USB.
Next step is to replace logic board. But might as well buy a new laptop at that point.

The spinning beachballs and strange behavior are similar to what you're seeing.

So, perhaps try loading system down. Render a video. I believe I chose Prime 95 as it didn't access drive much. Mostly CPU intensive.
Not sure if any of this helps.

Thanks for the reply, I very much appreciate it. I have heard previously that the HD and the logic board may be the issue. I will try testing this, although I'm putting it down near the bottom of list, not because it isn't likely, but because it is hard and expensive :) I want to test the easier stuff first.

Apologies for the noob-level questions. Thanks for any responses!

Running in Safe Boot, it's clear that none of the User Launch Agents are the issue. So what is left is the other launch and startup items listed below.

I'm not clear what you are saying here. A safe mode boot stops ALL launch items, whether they are USER items or system items. So just to confirm, are you still saying things work fine in safe mode, but not in a non-safe mode regular boot? If that is the case, it is proof positive something on this list is the problem.

I assume that I ignore the system processes. For Launch agents/daemons/user agents: I can locate these in USER/Library and /Library. How do I shut them off? Is it a matter of deleting the files and/or their apps?

You can ignore anything from Apple, and Etrecheck already ignores those. We are after third party, non-Apple items.

And are Other apps (just running processes, but some of them are running without me initiating them)/Internet Plugins/3rd party Preference Panes in the discussion as well?

You can ignore Internet Plugins. That would cause issues with nothing but your web browser, and that is not the issue here. Pref panes won't cause an issue either, but a launch item associated with a pref pane might... like the Java helper tool launch daemon running there.

OK, thanks. Yes, the mahine runs just fine in Safe Mode, and runs just fine in Safe Mode with the six startup items that were originally in my Preferences as startup items. Will get started booting normally with the other stuff on the list disabled tonight.

You've received some exemplary assistance from posters here but my experience with 2011 iMacs is the original hard drive & graphics card fail after four to five years. Moreover, my own testing ( Disk Utility and various other utilities ) didn't conclusively identify a problem. However, Apple's diagnostics, which they ran right in front of me at the Apple store, identified both the HD and graphics card issues even though the machine was not exhibiting any visual symptoms the day it was tested at the store. btw: I've found the easiest way to transport an iMac from the car to the store's interior is via a luggage bag with wheels and a pull handle. Good luck. Please let us know what happens.

I have replaced the HD on this machine, but I will add the video card to the logic board (and memory and HD as possible hardware issues.

I have pulled everything out of the three Library/Users startup items folders. Ran that for three days. No problems.

Rebooted today into normal boot with the six User/Accounts apps I had traditionally used.

I will let that run for a couple of days and then turn on Time Machine.

Then restore the three launch daemons/agents folders from the desktop to the root Library and Users/Library folders 72 hours apart to find the transgressor (I hope)!

Turned on TIme Machine tonight. It all seems good.

Sunday I will reboot after restoring items to the User/LaunchAgents folder.

Library/Launch Demons do not seem to be the culprit: next up are Library Launch Agents.

(Time Machine and my 3rd party launch items in Settings/User accounts also past the 72-hour fresh reboot test!)

I had the same simptoms as you are describing, played with it for many weeks, and finally broke down and took it into the store. They didn't find anything themselves, but replaced the HD and cable just on spec, and because of the detailed documents I was keeping of the issues.

Haven't had an issue since......I'm betting it is the HD

I am not sure if this is a case of no bad news is bad news, but . . .

Ran for two weeks in Safe Boot mode, no beachball crashes.

Ran for 72 hours after restoring my third party launch apps to the User/Accounts tab in Preferences

Ran for 72 hours after restoring items to {User}/Library/Launchagents

Ran for 72 hours after restoring items to Library/LaunchDaemons

Ran for 72 hours after restoring items to Library Launchagents

No crashes, no beachballs, nothing. So it's fixed? Only problem is: no culprit located!

Thanks for everyone who helped. I will certainly report back if and when the beachball freezeouts resume.

Hi.

I have similar problems after the update on the Mojave. Periodically (several times a day) all applications (one by one) start not responding and "beachballing". So only forced shutdown helps with a long press of the power button.

The system was completely reinstalled, but the problem did not disappear.

After a long study of the forums and my logs, the spindump process was suspected. (Killing spindump process did not help)

Then I tried to restart OS in Safe Mode and all was fine. But after Normal mode the problem return.

I uninstalled istat menu - it didn't help.

What to try next?

Can someone advise a solution? Thanks!

Then I tried to restart OS in Safe Mode and all was fine. But after Normal mode the problem return.
This is a big clue to the problem. Safe mode stops all launch and startup items from running, so if safe ode fixes it, that tells you it is one of the launch or startup/login items on the Etrecheck list causing this.

I see from the Etrecheck report you have a LOT of launch and startup items runnings. The only way to trouble shoot this is by removing them and testing until you find the culprit.

I would print out the Etrecheck report as use that as a worksheet then remove half the items at a time and restart until you can narrow it down.

Look in all these folders for items from the report.

After a few tests I noticed that Mojave works good with no sleeping (about 2 days).

But when I activate screensaver for a night and log in at the morning - all the application start no response and beachballing.

"But when I activate screensaver for a night and log in at the morning - all the application start no response and beachballing."

I realize this sounds simplistic, but why not just shut it down (off) for the night, and reboot the next morning...?
