I used the Health app to access my local medical network that administered the vaccine and gave them access to the Health app. It added my vaccine shots to the app, but no way to transfer them to the Wallet.
 
On the health app: browse > slide down to Immunizations > if present choose CIVID-19 Vaccine > choose your shot ie: Moderna COVID-19 Vaccine > select View in Wallet

Log onto the MyChart first.

If your healthcare provider uses MyChart (Epic):

  1. Log into your MyChart account on the web or via app.
  2. Go to the Menu and select COVID-19 under My Record.
  3. Select QR codes to generate a QR code or Download/Export to generate a PDF copy of your vaccine results.

If you received a QR code​

You can scan a QR code given to you by your COVID-19 vaccination or test provider to add your verifiable health records to the Health app.
  1. Open the Camera app from the Home Screen, Control Center, or Lock Screen on your iPhone or iPod touch.
  2. Select the rear-facing camera.
  3. Hold your device so that the QR code appears in the viewfinder in the Camera app. Your device recognizes the QR code and shows a Health app notification.
  4. Tap the Health app notification.
  5. For verifiable vaccination records, tap Add to Wallet & Health to add the record to the Health app and Wallet app.
  6. Tap Done.

Kewl….I had to re-enter some account info, but was able to scan the QR off my computer screen. Worked. Thanks!

In Alberta and had to do the same.
Ontario: Just recently got the booster and added the QR code to Health, where my original 2-shot was. Was then able to add the new one to Wallet and delete the old one.

That said, yesterday a business used its scanner on the QR in Wallet and it couldn't 'see' it. Fortunately, I had a photo of the original confirmation and QR code on my phone which I pulled up - and that it could read. So clearly worth keeping both on your phone! ?
 
Oh, how I wish the UK was taking more advantage of the health app! Only 2 NHS areas using it, neither are mine. ☹️
 
Is anyone here from NY State who has received booster shot, but the newly retrieved Excelsior Pass Plus is still not showing it?

I received my booster more than a month ago but still no show in Excelsior Pass Plus. Yes, I did try retrieving a new copy several times.

Mine showed up ok. Check the place you got the booster from to make sure they've sent it to the state database.

Well, if you were unfortunate enough to get vax’d in Oregon such as me then &h/t out of luck, because Oregon is still in the Stone Age when it comes to any type of electronic efficiently due to the fact that Oregon is in a perpetual state of bureaucratic quagmire governed by narrow minds.
 
For me, I found the QR code by accident, I was in one of my medical apps, I just happen to be looking for some other information. Was able to add the first 2 shots. However, I was not able to add the booster as the place where I got the shot is not participating in the Apple Health immunization records. What I did was snap a picture of each COVID shot record, create a Shortcut to display the photos. I thought it might be good to have a record of all the shots in a easy and convenient way.
