Looks good, the driver loaded, so I think it will be compatible with the 5,1 firmware.

First, download this version of UEFITool:
UEFIPatch_0.25.0_mac.zip 1.38 MB

Save you firmware, and open it in UEFITool, you can dump your Firmware with @dosdude1 ROMTool.

ROMTool.zip

In UEFITool File>Search chose GUID and search for:

Expand that GUID out until you see PE32 image section.

Right click on it and choose replace body. Open the AppleAGP.efi, then File>save image file.

That should be it, now just flash that firmware.

Reboot to a shell as we did before, or if you have a GOP card, install that with your GT120 so we can see if the driver loads for both cards.

If the AGP driver doesn't break anything, and you get to the shell on your GT120, upload a drivers.txt file for us.

EDIT:Just noticed you have a Radeon GOP card, but you need to flash a valid ROM back to it, you must have edited the VBios protected tables. That won't work for GOP boot screens, as the driver never links to the device>INVALID.

I'm gonna see if I can get GOP firmware on my 560, I've read it might be possible, if so I'd like to join the fun! lol

I saw some errors as well, but could not redirect them to a file.
I tried :

Not sure about the errors, maybe a pic of the screen?

Your drivers.txt showed the driver loaded, but it wasn't linked to anything as the AGP driver needs to be loaded before the system scans the PCI cards for option roms.

I really wouldn't over think it, so long as you have your MATT card and know how to use it to recover from a bad FW flash. I'd just go ahead and replace the driver with the older one from the 3,1, and it will work as we expect, or it won't.

Not too much else we can try, at this point.

EDIT: Piping the load command seems to work for me:

I have changed the part number only. But I can download the original vBIOS.

You won't get the Boot Screens without a custom boot loader.

However, the AGP Driver is not loading for your AMD card:

I'm not sure how the MATT card works, or what you did when you flashed your FW. I assume the MATT card will always boot the FW on the MATT card, not the one on the logic board?

What tool did you use to flash the patched FW?

I'm not sure if you have the MATT card installed, if flashing the FW flashes the MATT card or the logic board?

Can you remove the MATT card, flash the patched FW to the logic board, then reinstall the MATT card to recover only if the patched FW doesn't boot?

The MATT card overshadows the original ROM. I have tested that already. For the system that is the active ROM. I flashed with the ROMTOOL. It was a fast flash maybe because everything is the same only the AGP driver was different. Anyway, the Romtool declared success after flashing (it took only 3-4 seconds). I don't want to flash my original ROM yet, because if I brick it I have to desolder it to reprogram so I keep it as a fallback ROM.

I don't think I reflashed it properly the first time so I repeated the procedure. Now it took some time before the result came up. Next I blessed the Bootx64.efi and upon reboot a verbose output came out on the screen after which an Apple logo with a progress bar, which never initiates (same as when you upgrade the firmware the Apple way). It sits at this screen and I don't know how to proceed now. Should I reboot?

Not real sure what's going on there, give it some time, if you don't see any progress after 5 or 10 min., you won't have any real choice other than to reboot.

If you reboot, hold the option key and try to boot the OS, not the shell, and see what that does.

I assume what is going on here is you have Mojave installed, and the system is booting Mojave on your GT120.

That won't work.

Try and zap the pram, Comand+OPT+P+R three chimes.

As @h9826790 says, the blank screen is likely the sonnet card, you may have to remove it to get to the Apple Boot Picker. May have something to do with a conflict between the older AGP Driver and the Sonnet card.

When there is a bootable drive installed on the TempoSSD card, the boot manager may not able to show up properly. It could be only some options show up. Or completely white screen with no option at all.

Performing PRAM / SMC reset with empty card (no drives installed) may able to fix this issue temporarily (usually will come back after any drives swap / change. Or BootROM flash. Or any PRAM / SMC reset).

For this GOP test, you better completely remove this card to avoid false result.

This is the first time I got a white screen. Never had an issue booting from it with bootpicker. The problem is that HS is on the card

I am copying HS on a HDD and will remove the Sonnet card. BTW tested Next loader and there was no boot screen

That's sad news, I assume you can't get to a shell to see if the AGP Driver loaded to both video cards.

Before you flash a good FW back to the MATT card, you may as well see if you can get boot screens with this custom version of Clover.

I tried blessing bootx64.efi and later cloverx64.efi. The verbose output worked, but it never stopped at a clover screen just rebooted straight to HS. It did't ever create a debug file:

If SIP is enabled you can't use the bless command to set the boot loader.

Likely just time to call this one a failure and flash a good Rom back to your MATT card, assuming you can boot Recovery to disable SIP.
