I know you said Time Machine is off, but try running this in Terminal to make sure there are none left behind.

tmutil listlocalsnapshots /Volumes/

It showed no local snapshots? I'm running out of ideas here. That du command should show everything on the drive, even hidden and system files. What do Finder and Disk Util show for space used?

It shows 251GB and only 11.46GB available.

Yes exactly. So that shows almost full also but the du command does not. I'm really stumped here.
