The GM is usually finalized and loaded onto devices 2-3 weeks (at the very least) before the event.
Yes there was, when it didn't come out Monday or Tuesday.

GM is not a beta, and the GM is always after the event. iOS 12 was, as per my earlier post, iOS 11 was released on Sept 12th, with the iPhone event being the same day. iOS 10 GM was 8th Sept with the iPhone 7 event being Sept 7th. iOS 9 GM was 10th Sept with the iPhone 6s event being Sept 9th. iOS 8 GM was 9th September, with the iPhone 6 event being the same day. iOS 7 & the iPhone 5s event both on September 10th.
Therefore, there's enough track record for iOS GM to be out on same day, or within a couple days after, the new iPhone event.
