Icons are Flurry icon set. Can be found at The Iconfactory. Dock is Official Snow Leopard Dock BLACK, I got it somewhere in deviantart I don't recall where exactly though. A Google search should show it up

I use Candybar to change icons. It's compatible with iContainers. LiteIcon also works, and is free.

There are various programs, such as SuperDocker, for changing the dock.
