I've not seen it here in the UK before, I've driven in to work this morning with my phone plugged in (iOS 14.7.1) and didnt have the option.Everyone in the US has been able to report since mid-March (14.6??)
Nope, there is a chance its not even on the latest version of iOS14. She didnt even notice she had the option, i was driving and noticed the icons looked different down the side and saw the report hazard option.Is her phone on the iOS 15 beta?

Only spotted in on her phone in CarPlay to date, not actually look on the phone as such.That’s why I said US…I forget where everyone is from. Hah.
UK just got it last week, but should work on both 14 and 15 since it is still part of the 14 release. But maybe it is in limited testing right now? Not sure why though.

Well...appears something has filtered through between me coming into work this morning and heading back out just now. Can now see the option to report on both my phone and in CarPlay.
Sorry…you are correct about it not being on the phone screen, but in iOS15, you can swipe up on the bottom menu and press “Report An Issue.” This brings up another menu that let’s you report an incident even when not routing.I'll double check shortly when i pop out but i'm sure on CarPlay the icon to do it was visible in the same place whether i was routing or not.
On my actual phone i cannot see an icon for it at all whilst not routing anywhere. Once you start a route as you say you can bring up the menu from the bottom to report something.
