MP 1,1-5,1 Bluetooth does not work after installing Ventura with OCLP.

In your link before, where I can see the wiring for the adapter, there is only D+ and D- but you said we need to connect all 4 pins. On most adapters I have seen on ebay and my own one I can only see D+ and D- pinouts. Can you give me a hint where I can get ground and +5v from?

That was specifically to connect the factory BT module to a USB v2.0/v3.0 hub. not a BCM94360CD + adapter.
OK, so do I only need to connect D+ and D- to an external USB v2.0 port in the configuration with BCM94360CD + adapter?
+5V and GND are provided by the mini-PCIe adapter to the BCM94360CD card directly, there are only D+ and D- on every format adapter that I know of.

I've found a complete workaround for the cMP 4,1 and cMP 5,1 machines no longer accessing Bluetooth after installing Ventura.
This is going to be more complicated to write about than the actual install. I am a Photographer who uses my classic Mac Pro 5,1 machines daily for Photo Editing but also a retired US Navy Data Systems Technician. I know my stuff when it comes to researching.
I am not associated with any of the sellers or manufacturers referred to here. I reference some links that I am also not associated with. This is provided only for reference and I am not responsible if you damage your computer, lose any data, etc...
Remove the wifi and bluetooth cards completely from the cMP. The instructions for doing this are available with a google search online.

We will not be reinstalling the stock modules. Period. *Ignore the rest of the tutorial other than removing the modules.*
Insulate the antenna cables with electrical tape to prevent shorts to the motherboard.
Install a FENVI FV-T919 PCI-E Wireless Adapter. Use the included cable with the FV-T919 plus an adapter cable from standard PC Motherboard USB header pins to USB A to connect to a USB 2.0 or USB 3.0 port for Bluetooth. I ran the adapter cable out of a blank card slot and plugged it into an external USB port. Others have successfully run it to an internal USB A port on a PCI-E USB 2.0 or 3.0 card.
I pieced this together from other posts here plus a lot of research online. The FENVI FV-T919 uses an Apple brand controller so it is natively supported without any drivers required. It supports Continuity, Handoff, AirDrop and Bluetooth 4.0, etc.
I did have to do a clean install of Ventura to get the WiFi working as it continuously searched for WiFi when I tried to run it on my system that I had upgraded from Monterey to Ventura. After the clean install it works very well. I also bought external antennas to test out as I had read that there were some issues with the included antennas not providing strong enough signals. Your mileage may vary but I have tried both the original and the external sets of antennas with good luck. I have this setup running now on my 4,1 -> 5,1 machine. Please note that I have all firmware updates current on the Mac and it has 64GB RAM, 3.46 GHz 6-Core 5690 XEON, etc.
It is working so well that I'm doing the same install on my main workstation 5,1 Dual Processor with 5690 3.46 GHz processors and 128GB RAM.
I found the needed parts on Amazon and had the system up and running again in a day.
I personally chose these parts based on my own research and can vouch that they work well for my particular installation. Once again, your milage may vary.
I hope these images are self explanatory but you will need to order 2 sets of the external antennas as the FENVI FV-T919 card has 4 antennas ports on it.

First of all thanks a bunch for posting these instructions. I have been using the following upgraded card in my 5,1 to get hands off and Airdrop etc. It was very neat as it was 1:1 replacement with the original card. Is there anything like that? or any instructions how to covert it to make it connect to USB.First post here but a longtime Computer Technician.

Is there any way I can connect your provided solution internally so I don't have to run cables outside of the machine? MY PCIE slots are full so trying to figure out how can I make things work without pulling something out.

This was discussed already on the thread - Fenvi T919 is just a BCM94360CD in a nice standard PCIe x4 adapter.
This proposed solution does not solve the issue with MacPro5,1 OHCI controller support being removed from Monterey, you still need a powered USB2.0 or USB3.0 hub to force EHCI and workaround the removal of OHCI support by Apple.
If you connect your BT controller to a PCIe v3.0 card instead of the USB2.0/3.0 powered hub, it will work, but you won't have any BT mouse/keyboard enabled before the moment macOS loads and configures the USB3.0 drivers, very late in the boot process, so, you won't have boot picker working with your BT mouse/keyboard.

Why? The Fenvi uses the same BCM94360CD chipset as the iMac card used to upgrade the Mac Pro in the very guide you are linking to. Why would anyone waste a full-size PCIe slot and an USB port when a stock-like solution is readily available?
That's fine you did the research, however we have this topic covered extensively with a list of adapters available. And it's butt ugly, not to mention a waste of the available slots. Most users with hardware powerful enough to run Ventura use a >2-slot GPU and NVMe and USB3 controllers, so space is at premium in a Mac Pro. There is a dedicated slot and well-documented procedure for upgrading the legacy wifi/BT to current standards without any trace of modification in the 5,1.
Can you please point me to step by step instructions how to convert previously upgraded OSX wifi to work on Ventura or any clean internal solution that someone may have figured out. I don't want to give up on my hands off, unlock with Apple Watch etc features by using a dongle. This is only thing that is stopping me to move forward.

You need a powered USB 2.0 or 3.0 hub, not a new/different PCIe USB controller card. If you do it this way - btw this controller you linked can't be installed to a Mac Pro since it's PCI and not PCIe - you will only have keyboard and mouse working on the macOS login screen.
Connect all you keyboard/mouse/BT controller to the powered USB2.0/3.0 hub, the USB hub to one of the Mac Pro NATIVE USB2.0 ports. That's the only way to get Monterey and Ventura support for BT and HID devices from the moment you power your Mac Pro.
Please re-read this thread from the start, I've explained this several times, also read the OCLP documentation, this is clearly mentioned there.

Do you even had some thought about the implications of that? No BootPicker, no Single User Mode, no easy way to get verbose mode and etc.
You sure can live without all that, but when your Mac Pro have a problem and you gonna need some of what I've listed, you gonna regret hard not doing it the right way from the start.
Buy a powered USB 2.0/3.0 hub and then you search for the cables needed.
The cable you linked only works for a Fenvi T919 card. You will need several adapters to connect to the AirPort Extreme BCM94360CD.
The way to correctly solve the removal of OHCI from Monterey/Ventura is to force EHCI via a powered USB 2.0/3.0 hub:

Ok, I will order a powered BT 3.0 hub and plug it into native 2.0 port. Can you point me to any post that can guide me how to connect upgraded AirPort Extreme to external hub? And guide that can tell me which conversion cables to buy?
I'm feeling that I'm writing the same thing three times already, let me repeat again what I've written before on this thread: AFAIK, no one did a step-by-step guide for this, since this is very new and there are too many moving parts.
You can make a guide yourself, btw.
For the cables, when I have it on hand and tested I can link to what really works without the usage of multiple adapters, until then I'm not gonna link to anything.

Thank you for your input. As I stated in my original post, I found a COMPLETE workaround...Remove the stock modules and install the Fenvi FV-T919 instead. That's why I only referenced the guide to help with removing the stock modules as you can see. Even with my 2-slot GPU, my USB 3 controller card and my OWC ssd controller card in each machine I still had a vacant slot available.
In my 4,1->5,1 and my 5,1, I needed to get back into Photo production with Bluetooth available for my Wacom tablets in Ventura as soon as possible and this method worked well for me.

First of all your post is useful and important so please keep it. I may use it because no one knows yet how to connect upgraded Airport card to external usb. You are very helpful and awesome person. Can you confirm that air drop & unlock with Apple Watch works with your method?
Airdrop works from my iPhone 12 Pro Max. I don't have an Apple Watch to test it with.
