On the adapter you can see which wire color is + or -

@pdxplmI'm not sure what I'm doing wrong, but I'm using OLCP 62 and Ventura on a CMP 5,1. Initially during setup I had to use a hardwired mouse and keyboard, but after that the rest of the installation I had full BT capabilities.
I did a clean install with OCLP 62, latest Ventura, and stock card. I had Monterey on it previously, wiped it clean and then started with the latest release of Ventura. The initial install was looking for a BT keyboard and mouse. I had to use a hardwired keyboard/mouse. The install went seamlessly after that. I then connected all my wireless/BT items w/o any issues.
I read release notes for OCLP 6.2 and there are no changes in regards to Mac pro 5,1 and older BT updates. It appears we still need to use external hub and plug BT to it. I am using Rx-580. It’s not flashed. Everything works with Monterey. Based on my understanding Ventura has dropped old BT drivers and hence as it’s plugged on motherboard it’s not working. I am planning on external USB hub and building a cable as shown above in diagram in earlier threads. The diagram you posted above is what I am gonna follow.what kind of GPU your using? Hi, I installed this week with a clean install, OLCP 62 and I have an AMD RX 580 that is not patched.

I've found a complete workaround for the cMP 4,1 and cMP 5,1 machines no longer accessing Bluetooth after installing Ventura.
This is going to be more complicated to write about than the actual install. I am a Photographer who uses my classic Mac Pro 5,1 machines daily for Photo Editing but also a retired US Navy Data Systems Technician. I know my stuff when it comes to researching. I am not associated with any of the sellers or manufacturers referred to here. I reference some links that I am also not associated with. This is provided only for reference and I am not responsible if you damage your computer, lose any data, etc...
Remove the wifi and bluetooth cards completely from the cMP. The instructions for doing this are available with a google search online.

We will not be reinstalling the stock modules. Period. *Ignore the rest of the tutorial other than removing the modules.*
Insulate the antenna cables with electrical tape to prevent shorts to the motherboard.
Install a FENVI FV-T919 PCI-E Wireless Adapter. Use the included cable with the FV-T919 plus an adapter cable from standard PC Motherboard USB header pins to USB A to connect to a USB 2.0 or USB 3.0 port for Bluetooth. I ran the adapter cable out of a blank card slot and plugged it into an external USB port. Others have successfully run it to an internal USB A port on a PCI-E USB 2.0 or 3.0 card.
I pieced this together from other posts here plus a lot of research online. The FENVI FV-T919 uses an Apple brand controller so it is natively supported without any drivers required. It supports Continuity, Handoff, AirDrop and Bluetooth 4.0, etc.
I did have to do a clean install of Ventura to get the WiFi working as it continuously searched for WiFi when I tried to run it on my system that I had upgraded from Monterey to Ventura. After the clean install it works very well. I also bought external antennas to test out as I had read that there were some issues with the included antennas not providing strong enough signals. Your mileage may vary but I have tried both the original and the external sets of antennas with good luck. I have this setup running now on my 4,1 -> 5,1 machine. Please note that I have all firmware updates current on the Mac and it has 64GB RAM, 3.46 GHz 6-Core 5690 XEON, etc.
It is working so well that I'm doing the same install on my main workstation 5,1 Dual Processor with 5690 3.46 GHz processors and 128GB RAM.
I found the needed parts on Amazon and had the system up and running again in a day.
I personally chose these parts based on my own research and can vouch that they work well for my particular installation. Once again, your milage may vary.
I hope these images are self explanatory but you will need to order 2 sets of the external antennas as the FENVI FV-T919 card has 4 antennas ports on it.

Just to be honest, I like to keep things internal and will connect to internal USB port on a PCIe card. I will get a powered USB hub and plugin a wired USB Keyboard and mouse and keep it under my desk.EDIT:
It was pointed out to me that a powered USB 2 or 3 hub is required to be used between a Mac Pro external USB 2.0 port and the Fenvi FV-T919 Bluetooth USB cable in order to have Bluetooth mouse and keyboard function immediately from power on. I tested this configuration with an external powered USB 2.0 hub and it works as I was told. (Please disregard the reference in my original post about using internal USB port on a PCIe USB 2 or 3 card as that configuration will not allow bluetooth keyboard, mouse or trackpad to function on bootpicker screen, etc.)
I see only 2 cables connected to the Bluetooth?A workaound for the internal Bluetooth board is hooking it to a USB hub by attaching an USB cable.
Notice that the original cable is not straight.
I just cut one cable I had from a dead backplane and soldered it to an USB cable.

Connecting the original Bluetooth module like this is ill-advised because the standard USB connection provides 5V while the module is designed for 3.3V (VCC in your diagram) as provided by the internal header.
Thats the way it works if you want to get rid of USB 3 (and a TitanRidge btw).Here are the steps to get BT working
1. Install latest open core patcher
2. Remove if you have any BT 3.0 cards
3. Clean Install using usb
4. Apply root patcher on boot and make sure it shows usb 1.1 patch was applied
5. after restart you should have BT without any modifications.
Can anyone test ? Based on YouTube video BT 3.0 PCIE interferes with patch for 1.1 usb
Keep in mind if you install usb 3.0 card back after root patch Mac will refuse to boot. So basically either choose 1 or the other.

Sorry for confusing you, I updated my post as well. You need to remove if you have any PCIE USB 3.0 card(s). There is no need of unplugging Bluetooth card. Original BT card is plugged internally on motherboard on 1.1, so it’s not the problem. Buy externally powered 2.0 usb hub. After installation, when you install root patcher make sure it shows USB1.1 patch installed. Don’t install PCIE USB 3.0 card back. There is no physical modification needed to BT module of machine.BT 3.0 cards? Does this include the BT original card remove the connector while installing Ventura? Is a clean install necessary can you revert root reboot then re-install root package then re-connect original BT cMP if removing cable to original BT card is necessary during install of root package?
I do have a USB 3 card installed on PCIe but not powered still I will remove it.
