No, it is a software issue. The phone is allowing a FaceTime connection to be established without it being accepted. Has nothing to do with the FaceTime server.
I think that Apple is intentionally building in these bugs to keep the government happy, ie so they can spy on you. Otherwise how do you explain this complete incompetence?
Yep. Seems like a rather simple bug. They just test the cr@p out of it to make sure it works and nothing else is destroyed in the process. Imo...Both I hope.

I agree re the second part of your sentence (about getting it right). However, I would argue that Apple is "rushing" to get this done. This is not a minor feature (like the Stocks app). This is a major telecommunications related feature straddling the iPhone/iPad/ Mac platforms. Plus, its a black eye for them on privacy. They want this fixed ASAP. The pressure isn't from exposure (if they had left it active while fixing), its from not having a marquis feature unusable. I’m a little surprised that if the update comes today it hasn’t been announced by Apple. I know traditionally they don’t announce updates, but with all the press behind this I can’t believe they didn’t for this one.
