On main page of Macrumors someone said:

“iOS and iPadOS 15.4.1 fix an AppleAVD issue that could allow an application to execute arbitrary code with kernel privileges. Apple says this exploit may have been actively exploited, so it is a good idea for all users to update as soon as possible.”

I think it is a bit early… don’t you think? I mean, yes, technically we could only get some bug-fixing and stability updates in the form of iOS 15.4.2, or 15.4.3… But I would find it strange to not have any major update in more than 2 months. We’ll see.
It's so bizarre how people are convinced iOS 15 is finished. iOS 16 isn't coming out for something like six months lmao

Hmmmm, reboot iPhone and Apple Watch, still no OTA. Anyone else have a work around that helped?

Have you got a beta profile on your phone?

if yes, delete the profile and try again.

if no, go to automatic downloads, turn it off and back on, exit software update page and try again.

Hopefully this fixes all the weird behavior with CarPlay, like how I have to unplug and replug my phone at least once in order to get Siri to announce directions rather than just make a bing noise.

Try reboot as someone suggested

That’s the first time I see this popup on locked screen. Is it new or I am just today-years-old? :D

It says “Verifying the update”
Is it just me or does Polish sound like Portuguese? Definitely not written the same way though ?
