Did you turn off PowerNap for both Battery and Power Adapter? I don't know if it matters, but I turned off PowerNap using this command, instead of the UI:

sudo pmset -a darkwakes 0

Maybe try that and see if it works?
Yes, I did. It did not work.

After reading the many posts about this issue in the Apple Community forum, I noticed that a couple of people mentioned a dock in their post, and that they've had trouble w them. I have a dock. Mine is the OWC Thunderbolt 3 Dock, which I've now detached and I'm using the MBP charger. I've also connected my desktop monitor and an Anker USB-C hub to connect an ethernet cable. We'll see how that goes.

As I mentioned in my original post, my 2018 15" blew up a little bit over a week ago, right after the 10.15.4 supplemental update installed. Since I didn't know exactly how long it would take Apple to repair the 15", I bought the 16". As it turned out, the 16" and the repaired 15" arrived on the same day.

Prior to the explosion, the 15" had been plugged into my dock for the better part of a year. Not all the time, but much of the time.

I'll chime in with an update tomorrow. Thanks for your input.

I also use a dock -- from LandingZone https://landingzone.net/products/for-macbook-pro/for-the-16-inch-macbook-pro. However I have also read that docks can cause various issues.

Not using the dock did not work. Once I let it go to sleep, I could not wake it up. Had to use the power button.

So far, the only thing that's worked is to set the "Prevent computer from sleeping automatically when display is off". Period. I'm leaving that on until Apple fixes the problem, which was not happening before the 10.15.4 update.
That also worked for me. I have a Late 2013 15" MacBook Pro running macOS 10.15.4 Catalina. The crashes when waking from sleep started after that latest update. Checking the box "Prevent computer from sleeping automatically when the display is off" and also unchecking "Put hard disks to sleep when possible" has solved the crashing problem for me. I've never had "Enable Power Nap" turned on, so this wasn't the issue for me, unlike some other users.

I was able to replicate how it happens for me
I leave work, close MPB16 lid, get home connect MPB16 running 10.15.4 to Caldigit USB-C pro dock (lid still closed), eat, come back to MPB16 and it's dead.
Problem seems to be the dock or connection to a dock, I tried all 4 MPB16 TB3 ports.
Only have 2 dell u2412m connected to the dock at last test, no other accessories.

However, I just remembered I had that previously with my Mac 2015 - after reconfiguring/reinstalling several times and lots of "not politically correct" talking to apple ;-), ... at the end, it looks like: it was Dropbox/Google Drive extensions - just switched them off from auto-start - and now is my first day without restarts.

Will let you know if that will happen again - but fingers crossed ;-)

I have seen Dropbox listed as the first extension loaded in the crash reports posted by people so that could be the culprit for many people. Please do report back and please report this to Dropbox and/or Google if you do figure out which or both maybe causing the crashing issue.

Do you have dropbox or google drive or onedrive?

My status: 3rd day - no issues.

This is after disabling finder extensions - I think it was Dropbox but will test maybe over weekend..

I've been getting this issue with a 2013 generation Mac Pro that I recently did a fresh install of Catalina on. No dropbox, google drive, or one drive. The only way I fixed it was to disable all sleep options other than switch off the monitor.

Having the same problem with a brand new 16" MBP. I see a lot of theories about third-party apps, docks, connections to monitors, Migration Assistant, etc.

I have used none of those.

Sleep transition timed out after 180 seconds while notifying clients about upcoming system capability changes. Suspected bundle: com.apple.iokit.IOGraphicsFamily.

Same as @Awimoway. 16" MBP is a few days old and about as pristine as you can get. No external docks, monitors, hardware, etc. And I get the same IOGraphicsFamily:

Looking for an OSX "Crashalina" update pronto!

Turning off Power Nap for the power adaptor seems to have fixed the problem for me so far (battery Power Nap was already off by default).

Interestingly, a friend's new 2020 MacBook Air is also shutting down repeatedly, but we haven't captured the error report. Perhaps Apple has hired developers from Redmond.

You know there is no reason to bash Windows or Redmond because of your issues with Apple. The bottom line is Windows updates don't wreak nearly the havoc that Apple does and they will own to their mistakes. It always amazes me how people having issue with Apple then try and bash Microsoft. Hello?

My 16" also suffers from occasional crashes on wake. Is it the 16" or the useless Catalina... Don't care. Just not overly happy with this machine's various foibles.

Really hoping that Apple scraps 10.16, pushing it to next year so they can get the damn 10.15 working properly.

Have you seen the news about iOS's security bugs; all related to doing too much.

I think I solved it on my 16" MacBook Pro.

-Turned off PowerNap for both Battery and Power Adapter in Energy Saver Preferences
-Turned off Put Hard Disks to Sleep when Possible Energy Saver Preferences
-Entered Recovery Mode, ran First Aid in Disk Utility
-Entered Recovery Mode and performed a clean reinstall of Mac OS Catalina 10.15.4
-Installed Mac OS Catalina 10.15.5 Public Beta 4

No crashes for two days now. Fingers crossed.

That's not good. Almost worth calling Apple support about that; could actually be a hardware problem. Given that you've spent so much with Apple, the least you'll expect is a machine that works properly.

Hi all, same issue for me with a brand new Macbook Pro 16.
Regarding the usb-docks, just tell you all, that you are confusing "correlation with causation", the only thing it says that many of us (with this problem) has a dock or usb-c hub, is because Apple had the horrible idea of get rid of all the f. ports that is used by many professionals today (sd-card, usb-a, ethernet, etc).

So then the real causation of this problem most probably doest not have any realation with usb-docks or external monitors, otherwise we would suspect that all the problems has relations with usb-docks....

I have a 16" MBP and always used it on a CalDigit dock and 38" external display at the office with no issues. But I always took my laptop home at the end of the day. Saturday I moved the desk setup to my house and so far 2 nights = 2 crashes. Both times the machine was asleep and monitor in auto-sleep.
