Only the account holder can order / change phones.
TMO online ordering thru the App or website is best to avoid the fee but the account holder will need to be logged in.
I'd do the Apple.com route online and order it to avoid the $20 per phone service fee and account holder requirement.

Both me and the other person ordering are "administrator" level accounts. He is the primary, but my name/account is also listed to be able to edit and view all account information. In fact, I ordered a 12 Pro a few weeks ago this way.
That's only if you bought and traded your phone in with Apple. I had to use my own box and print the label myself for my T-mobile trade in. I used the outer box my new iPhone came in and put my old iPhone inside its original box first and then padded between the boxes.You will receive a separate box from UPS to send your trade in.
It has the return label and tape to seal it back.

You can use a different card and different address. Just needs to be addressed to the account holder, so keep that in mind when UPS label drops in the system. When did you originally order?
