Yes to both. It's in the third section led by iCloud. You can enable the menu item by checking the box at the bottom.Does the Bluetooth prefpane show up in System Preferences? If so, can you turn on (tick) Show Bluetooth in menu bar?
I got a same issue. I just got 943602CDP card to upgrade to 4.2 but when I installed it, no BT. So I go back with my old 94360CD card, BT working fine again. So I ordered another 943602CDP thinking, the one I bought was defect, and when I got replacement, same thing. No BT.
I don't know how that user post from page #743 got it working. He shows the picture and I have exactly same card.LOL same here
I just updated to 10.12.4 and faced some weird issue.
As soon as I rebooted, my BT icon shows not available yet my magic mouse and trackpad was still working. In system preference, no trackpad yet working. not fully but was enough to click and drag. Thinking 10.12.4 had to do something, so I wanted to run CAT to uninstall with recovery HD even though I did that before. So I rebooted to disable SIP using terminal csrutil disable in recovery mode, and when I booted back to desktop to run CAT, BT came back and working normally.
MacID still don't work. weird.....
Does anyone know where I can buy an extension lead for the bluetooth connector? I did find one on OSX Wifi, but with international shipping it works out rather expensive:

I have found similar things online, but almost no male to female leads.
Also, does anyone happen to know if the USB data lines have to be connected to the original connector or will any USB data lines suffice? I ask as I could connect it to the front panel, because I've replaced the original USB 2.0 ports so the lines aren't being used and it would just look a little cleaner.

It *looks* like it's right. Have you zapped the PRAM and reset the PMU?
Vigs - You have a 3,1 and most of this is about 4,1/5,1. If you just want wifi it is easier/cheaper to break down and go with a USB dongle. Also, I thought for BT you needed to tap the USB motherboard connector.
