I bought mine all ready to go from a HK eBay vendor so I don't really know I'm afraid. Sorry.
I tried finding it again on eBay but it looks like they've stopped selling them.

You'll need to use CAT to get that working reliably.
So I got this card (1st 4 columns are Vendor ID, Device ID, Sub-vendor ID and lastly Sub-device ID).
it sort of works (Wi-Fi keeps turning on/off every 2 seconds) and BT is not detected (System Profiler => BT).
any thoughts? Perhaps I got a non-Apple card? Thanks

I bought from this seller previously and my card didn't show Bluetooth either. Had to exchange it (took about 2 months) and only the second card from this seller worked.
You guys must be surgeons, wow was that a pain in the ass.
I succesfully put a BCM94360CD in my 3,1 but connecting those little leads was super painful. I could not get one of the screws to come out on the native bluetooth chip, but with one screw out I could pop it up and unseat it. Bluetooth did not work right away but after a restart everything seems to be going fine.
AC wifi and Bluetooth 4.0 for $35 not too shabby. Have not tested continuity and whatnot yet.
If I have my Mac hardwired to ethernet, should I also connect to the wifi to get continuity to work? Will I default use my ethernet for my connection?
edit: Maybe I spoke too soon. Im trying to pair my iphone and it wont connect. It says my device is not supported, and to make sure I have network access enabled?

I'm sorry for diverting, but after 35 pages, I'd like to know which is the absolute best way to update a 5,1 Mac Pro to have all the latest BT and WIFI features in MacOS Sierra that looks as stock as can be in 2017?

Previously I had used & was happy with the MacVidcards mod, a few years ago, & have now modded another Mac Pro 5,1.
I had problems with the MacVidcards mod - the card has been changed from what you see on the site - whereby the antenna terminals are on the underside of the card, which makes a difficult job nearly impossible. Additionally the cable to the bluetooth fitting was faulty, I've sent several emails with no resolution.
In the end I bought the OSXWifi card, which is similar to my original Macvidcards upgrade card. The antenna cable connectors are on the top which makes life a hell of a lot easier. Additionally the connector to the original bluetooth point is much better.
Additionally they have solutions for more Mac types.

You won't be able to send SMS text messages though.
For the people stuck with using SMS, they tend to be unreachable via the Mac.
I used to be able to do the same thing with my Mac Pro 5.1 (2012) OOTB! But, all of a sudden I can SEND but not RECEIVE any text messages anymore. Any idea?

Yes. But who uses SMS these days? I don't get them anymore. It's iMessage or Whatsapp.
There's a few people that do not have iPhones nor Messages. Straight up PC folks, and available via text message only. Those people are also important in my life. It's weird having to be around my iPhone to get their messages instead of it popping up in MacOS like everyone else. On top of that, I cannot send messages to them either. It kinda sucks.

I'd like to be completely aware of all communications brought forth of my iPhone with the Mac. It seems like this upgrade allows that. If you don't need it, fine, but I'd be surprised that without this upgrade if EVERY message comes through on your Mac?

Continuity not even require WiFi, I can use my Mac to take phone call, SMS, etc before I install Wifi card (only cable network available).
I just did one of these installs on my 3,1 with an eBay card with the ports on the back of the card. Serious pain in the ass and could not have done it without long tweezers and patience.
I had to install them right side up, then flip it over, install it into the adapter, and get it in. One of the leads was really short and wasn't sure I was gonna pull it off.
I recently bought a GMYLE Bluetooth USB dongle as recommended here (http://www.macworld.co.uk/how-to/mac-software/get-continuity-handoff-airdrop-on-old-mac-3582632/). Plugged it into my Mac Pro 5,1 and followed the steps for the Continuity Activation Tool (https://github.com/dokterdok/Continuity-Activation-Tool) which I understand hasn't been updated for over 12 months.
Followed all of the instructions including disabling SIP.
On restart, heard some strange high pitched buzzing sounds. OS X failed to boot, so ended up restoring OS X.
Tried the same thing again after the fresh restore.. and still failed to boot.
I looked at the 'Issues' link and the most relevant issue appears to be #399, titled "Failed boots after CAT install on mid-2010 MacBook Pro macOS Sierra 10.12.5 #399" but the Wiki page is blank..
Anyone had any similar issues? Thanks.

That setup will only get you AC Wifi. The PCI slot that the airport card fits into on the MacPro5,1 does not have USB on it that is required to enable the Bluetooth portion of the card you have. From the looks of it, the two solder pads next to the label R1 are the data channels for USB. On the card I bought from EBay these pads were soldered to a wire that I could connect to the USB port the original Bluetooth card used.
