V100HalWeb is the VOLTA web driver, which is only available in 387.10.10.15.15.108.
The last modified dates do not line up with a recently installed driver being used, however I'm not 100% positive if these dates are from the 387.10.10.10.40.113 installer (which was released 12/10/2018), or if they were just installed at that time on your system.

Sort by version column and look at all the 10.33.0 versions and you'll see 12/7/18 @ 5:07AM / 5:22AM dates and times.
Can anyone on the 387.10.10.10.40.113 drivers double-check your last modified dates?
NONE of the NVIDIA web drivers are being loaded. Apple's KEPLER driver is being loaded (which is baked into the OS). If your GPU is a Kepler-based TITAN, that would at least make sense why you can function without an EFI.
Other things to try:
Use an EFI GPU. Completely remove the NVIDIA web drivers and CUDA. Shut down machine. Enable SIP. Enable Gatekeeper. Start machine from cold boot. Reinstall NVIDIA Web Drivers (without NVIDIA TITAN installed). Keep an eye on Security & Privacy tab. Shut down. Take EFI GPU out of machine. Install TITAN GPU and ensure it is properly powered. (ONLY HAVE ONE GPU IN SYSTEM!)
Attempt 387.10.10.15.15.108 if you're still having install issues.
