Do you have the folder
/private/var/folders/zz/zyxvpxvq6csfxvn_n00000s0000068/C/softwareupdated/com.apple.SoftwareUpdate.SUCatalogDataManager/z/031-71809 on your system? (if yes, then the patch tool should be able to update it) - when you press "Patch Update" in the tool what do you get?

Has SIP been disabled on your system? (command is csrutil status in terminal - if it says enabled, you may want to boot either in recovery mode or using the USB patch drive and going into terminal and typing csrutil disable). (this will make it easier to patch your system and apply kexts). If you then boot from the USB patch and re-patch your system (and patch your recovery partition if you didn't the first time around), that may fix your update woes.

Failing that, PM me and we can have a discussion and try a few things.

Hmm. SIP likely isn't holding it back, but nice to have it disabled anyways.

If the tool was able to patch the update successfully, then, honestly, you likely just need to be patient and wait for it to show up in the App Store updates (Apple's servers could be busy as well).

Looks like the Recovery HD update did load right after installing the PB. But nothing else shows up...

Well, if nothing shows up after leaving it for about an hour, maybe reboot with USB patch, disable SIP, reboot again with the USB patch and re-patch your OS and the recovery partition (if you didn't patch it yet). It can't hurt anything by re-applying the patch and could fix any issues if some files got updated along the way.

@dosdude1: What did you modify to make recovery bootable? Did you modify something in the BaseSystem.dmg? When i add my board id/model id to SystemSupport.plist, and replace prelinkedkernel, i get a reboot after 4-5 minutes. I don't see any log when boot with -v.

You may need to rewrite your recovery partition, from my end the only things needed were to modify PlatformSupport.plist (which is probably what you meant)?

All the recovery patch does is set com.apple.Boot.plist to bypass the PlatformSupport check, and modifies prelinkedkernel for USB support. Nothing inside BaseSystem.dmg was modified, however making a modification to it may be necessary if the partition still proves to be unbootable.
 
Finally, I can update my machine (iMac Early 2009 with upgrade RAM to 8GB) from PB1 to PB4, after try and try and few tips of @dosdude1 and @fischersd (Thanks a lot to both) I had to do a new USB drive with Sierra Patch Tool (last version 3.5) and boot from there, after open Terminal and disable SIP, reboot the system into USB drive and I used "Post-Install Patch" with the option "Recovery Partition patch" ON. Boot in HD normally, I opened the Update Patch Tool and finally PB4 update patch is succesful, after that appears into App Store ready for download and install.

How can I disable sip from terminal?

You have to boot either from the USB patcher disk or the Recovery Partition, then go to the utilities menu, then terminal and type the command in terminal. You can't disable SIP while you're booted from the hard drive.

Yep, that'll do the trick.

A few people have needed the patch reapplied / permissions fixed in order to see the updates (or to have the updater patch work for them).

Disabling SIP also helps for a number of reasons.

I got sip disabled. When I run Sierra update patch, it says patch successful, but there is no available update in Mac App Store
 
Yesterday I can updated my machine from PB1 to PB4 but this morning when I started the computer the bluetooth and the USB drives doesn't works!!! Only works in recovery mode, in my user, the bluetooth logo has a "zig-zag line" and not recognize the mouse and the keyboard. Can anybody help me?
 
I tried everything with update patch about 200 times, restart, patch, permission, pram reset, cmd+r in app store, disabled SIP, etc... Nothing updates. mbp 5,1 pb3 to 4. Something useable information?

Re-run the Post Install patch. If that doesn't work, try it again, and select "Force Cache Rebuild" before rebooting.

Don't say "etc" if you want help. You need to say everything that you've done.
Did you use the latest versions of each of the tools? (did you make a new USB patch disk, with the latest tool, so it patches everything in the post-install patch?)

Oh, and if you actually do have the update working properly and actually hit Apple's servers about 200 times like you're suggesting, you may have been temporarily black-listed for hitting their servers too much. :) Once you have everything actually working, you need to leave it alone with the app store running on the updates tab and walk away...maybe for a couple hours.

Yes, I've the folder, yes SIP still disabled.
I already tried to download .pkg files manually and copied to /Library/Updates/031-71809 - Nothing -Képernyőfotó 2016-08-11 - 18.25.08.png
